On the TOEFL speaking section, I am having a hard time completing my answer. You don’t have much time so how can I manage to understand and respond clearly (for example on the question types that ask you to choose between two possibilities)?

You certainly do not have much time. But the most important thing is still to use a little time to PREPARE, otherwise you can loose track of the issue entirely or not arrive at your point, or even forget during the stress of the test to think of examples. So, take, for example, the TOEFL speaking question you mentioned which asks you to choose between two possibilities. On this question you should spend about 15 seconds preparing (writing down notes, examples, support) and 45 seconds actually speaking your response.
